Hip Osteoarthritis – Diagnosis and Treatment

Osteoarthritis is the wear and tear of the joint cartilage. Whereas in a healthy joint the cartilage distributes the loads, in osteoarthritis the cartilage tissue is damaged. This puts more and more pressure on the joint and the cartilage damage spreads. Aging processes and genetic predisposition contribute to the progressive degradation and eventual loss of articular cartilage. Hip joint arthrosis – also known as coxarthrosis – is the term used to describe the increasing wear and tear of the cartilage in the hip joint.

Since osteoarthritis is a degenerative wear disease, early treatment is necessary. Already damaged tissue can only be regenerated to a limited extent and later stages can no longer be healed. It is therefore all the more important to stop the progression of hip joint arthrosis and to alleviate already existing complaints before an artificial hip joint becomes unavoidable.

Symptoms of osteoarthritis of the hip joint

Typical symptoms of incipient osteoarthritis include an uncomfortable pulling sensation in the hip and morning stiffness (joint stiffness), which may indicate initial damage to the hip joint. As hip osteoarthritis progresses, symptoms intensify and expand to include start-up pain at the onset of movement after prolonged rest (sitting, lying, etc.), joint pain after weight-bearing, discomfort when descending stairs, and pain in the groin that may radiate down the thigh or into the knee joint. Later, limping occurs with prolonged weight bearing, as well as buckling of the hip on the affected side. Pain at rest and limited mobility may also occur as hip osteoarthritis progresses.

Causes of hip joint arthrosis

The cause of osteoarthritis in the hip joint can be very different. However, all causes have in common that they damage the articular cartilage and thus the natural function in one or both joints of the hip. As a wear and tear disease, the presumption of age-related wear and tear is obvious, but osteoarthritis in the hip can also occur at a young age if there is persistent overload due to physical activities, overweight or metabolic diseases.

Incorrect stresses and inflammatory joint diseases can also promote the occurrence of hip joint arthrosis. In addition, there are hip injuries due to accidents, deformities such as hip dysplasia, which increases high stress on the hip joints due to non-optimal movement patterns, as well as continuing stresses that individually or collectively promote the risk of developing hip joint arthrosis.

Diagnosis of hip joint arthrosis

For a careful diagnosis, the orthopedic specialist will ask you numerous questions about your medical history, lifestyle, diet, and family history. Via clinical examination, gait, posture, condition of muscles and musculoskeletal system are then checked to identify malpositions and physical characteristics of possible osteoarthritis in the hip joint. In addition, blood tests are performed and various imaging techniques are considered to determine the condition of the hip joint and possible cartilage damage. These usually include:

X-ray
Ultrasound
Magnetic resonance imaging (MRI).

Treatment for coxarthrosis

If the diagnosis reveals the presence of hip joint arthrosis, various treatment options are available depending on the stage of the disease and the extent of the cartilage damage already present. The earlier the diagnosis is made, the more patients can do themselves to prevent the progression of the degenerative joint disease. Light exercise, a healthy diet, reduction of existing excess weight, massages and physiotherapy or the use of orthopedic aids (e.g. orthopedic insoles against pelvic obliquity and postural deformities) can be the first steps towards alleviating the symptoms, in addition to drug treatments for active hip joint disease (if inflammation and joint swelling are present).

More advanced stages may benefit from transcutaneous electrical nerve stimulation (TENS) therapy, if appropriate, or deformities of the hip may be corrected with surgery (e.g., osteotomy). Further interventions can serve to strengthen the cartilage or smooth out irregularities in the joint tissue. If all options for therapy have been exhausted, an endoprosthesis may be required as an artificial hip joint. For this purpose, however, the experienced orthopedic specialist will additionally advise you individually and personally accompany you from the operation to the aftercare.
